Getting Bacteria Through Your Stomach Alive

The Bifidus strain showed some resilience to bile salts even without the capsule’s protection, while several of the other strains tested lost most of their viable cells once exposed to the full force of digestive conditions. The takeaway: not every probiotic responds the same way once that protective capsule is gone.
So, what happens when probiotics are delivered in fermented food instead?
A 2006 study published in Applied and Environmental Microbiology tracked the fate of yogurt bacteria, specifically Streptococcus thermophilus and Lactobacillus bulgaricus, through the gastrointestinal tract by analyzing fecal samples from participants who consumed yogurt.2 By testing stool samples from participants after they consumed yogurt, the researchers confirmed that live bacteria from the yogurt survived the entire digestive journey from mouth to the lower gut. That kind of direct evidence matters in probiotic research because it moves beyond lab simulations and shows what actually happens inside the human body.
Researchers and scientists at the International Scientific Association for Probiotics and Prebiotics (ISAPP) note that studies suggest dairy products may offer enhanced protection for probiotic bacteria during gastrointestinal transit, and that the scientific community continues to investigate how delivery format influences real-world probiotic outcomes.5
Capsules vs. Yogurt: A Head-To-Head Comparison

Knowing that bacteria can survive the journey is one thing. Knowing whether survival translates into measurable outcomes is another. Two randomized clinical trials from Penn State University offer some of the clearest evidence available on that question.
A 2021 study published in mSphere enrolled 36 healthy adults in a randomized crossover trial designed to answer this question as cleanly as possible.3 Researchers compared the same probiotic strain, Bifidobacterium animalis subsp. lactis (a strain in the Bifidus family), delivered in two different formats: a yogurt smoothie and a capsule. The findings were intriguing. Participants who consumed the strain in yogurt format showed greater recovery of the probiotic strain in stool samples compared with those receiving the capsule format, suggesting that more of the probiotic survived the digestive journey, including passage through the stomach. Same bacteria, same amount, meaningfully different outcomes depending on how the probiotic was delivered.
What The Research Suggests About Immune Response
Another study from the same Penn State research group takes the story a step further, and this is where the science becomes even more interesting.
A 2016 study in Molecular Nutrition and Food Research found that participants who received the probiotic strain via capsule or in yogurt where the strain had been added before fermentation showed elevated NK-cell and T-cell activity, markers of a more active immune response, along with fewer days of upper respiratory infection symptoms.4 The one exception was a fourth group: participants who received yogurt with the strain added after fermentation rather than before. That group did not show the same benefit, a finding that carries real implications for how probiotic products are put together. Simply adding probiotics into a dairy product after the fact may not be enough. The research suggests probiotic bacteria may need to be grown through the fermentation process itself for the most potential benefit.
Taken together, these two human trials suggest that the way probiotics are delivered may influence not just how many bacteria reach the gut, but how those bacteria interact with the immune system once they arrive.
Why The Fermentation Process Itself May Play A Role
The 2016 Meng study highlights a detail worth paying close attention to. Among the groups tested, pre-fermentation addition and capsule delivery both produced similar immune outcomes, while adding bacteria to yogurt after fermentation did not show the same benefit. This suggests that it may not only be the dairy matrix that matters, but the process of being grown and embedded in that matrix through fermentation.
When probiotics are grown through fermentation rather than blended into a finished product afterward, they develop within the food environment they will eventually deliver. Whether that distinction contributes to better outcomes is an area researchers are still exploring, but the 2016 data points in that direction.

The question of whether the bacteria were fermented in the product or added after the fact may be worth asking.
